Checklist
Information Needed:
Name, address, and phone number
Pet’s name, breed, age and whether spayed or neutered
Veterinarian’s clinic location and phone number
Special instructions (allergies, medications, epilepsy, deafness,
etc.)
Emergency contact’s name and phone number
Immunization Requirements:
All vaccinations** must be administered by a certified veterinarian
or vet tech, and verified as such.
Dogs must be immunized against:
Rabies
DHPP (distemper, hepatitis, parainfluenza, parvovirus)
Bordetella
Cats must be immunized against:
Rabies
FVRCP (panleukopenia or distemper, feline rhinotracheitis, calicivirus
and pneumonitis-chlamydia)
**Any other variations must be verified by a certified veterinarian.
Things To Remember:
Make reservations early!!! Especially during seasonal events.
Use permanent marker or something to label your pet's items.
Let your emergency contact person be aware of your itinerary,
pet's needs, restrictions and other important information.
Make sure all immunizations are current and on file with our
staff.
Please do NOT allow any member of the family to stage an emotional
'farewell' scene. Your pets can be made to feel unnecessarily anxious
about the visit if they are subjected to this kind of dramatic display.
If your pet is not used to your being away, please bring in a
towel, tee shirt etc. (something with your scent on it)
Please try not to overfeed your pet or give excessive amounts
of treats before boarding; this may result in an upset stomach.
RELAX AND FOCUS ON YOUR TRIP OR ADVENTURE. (We are all animal
lovers and promise to do our best)
